I am a bilingual (English/Spanish) Registered Nurse with over four years of experience in case management, utilization review support, discharge planning, and interdisciplinary care coordination. I consistently complete OASIS documentation to support medical necessity and reimbursement while managing complex home health and ALF caseloads.

In my current role as an RN Case Manager / Clinical Manager, I lead discharge planning and transitions of care to reduce readmissions, communicate clinical updates with providers and agencies, and participate in case conferences, chart audits, and triage. I have hands-on acute care experience from progressive care and step-down trauma units, caring for medically complex patients in a Level II trauma center and collaborating on care progression and discharge readiness.

I hold an ASN and an AS EMS, certifications in BLS and ACLS, and EMT/Paramedic training. I am remote-ready, detail-oriented, and committed to patient-centered, compliant care that improves outcomes and supports smooth transitions across settings.
