What I'm looking for

I'm looking to continue leading care coordination and case-management work where I can support patients, collaborate with clinical teams, and ensure timely, guideline-compliant transitions of care.

I lead the UM Inpatient Case Management team at Riverside Medical Clinic, ensuring timely department work aligned with Medicare and health plan guidelines. I perform EOIC assessments using MCG criteria and coordinate safe discharges to appropriate levels of care.

Previously, I managed long-term care and case management for up to 100 patients under LPS conservatorship through the Riverside County Public Guardian’s Office. I coordinated nursing assessments, long-acting injections, labs, placements, and medical and social care across board-and-care facilities, while helping open and manage continued case management for Riverside MHRC, a 79-bed psychiatric facility.

At Riverside Medical Center, I coordinated care for 120 long-term-care patients as MDS Coordinator and previously provided direct care as Charge Nurse. I bring experience in MDS compliance, interdisciplinary care planning, chart audits, staff orientation, medication administration, crisis intervention, and skilled nursing care.
