I’m a highly motivated RN with seven years of hospital experience and strong utilization and case management background. I bring bedside clinical insight alongside Health Risk Assessments (HRAs), care planning, and coordination across complex member needs.
In my most recent role as UM clinical supervisor at Astiva Health, I conduct comprehensive HRAs for C‑SNP members and develop individualized, person-centered care plans aligned with CMS Model of Care requirements. I collaborate closely with the Medical Director on complex case reviews, clinical decision support, and escalation of high‑risk or rapidly changing conditions.
I facilitate Interdisciplinary Care Team (ICT) meetings, monitor utilization patterns, and support medical necessity review, prior authorization, and appeals processes. I also coordinate care transitions and discharge planning to reduce avoidable hospitalizations and improve continuity of care.
Previously, I worked as a Registered Nurse across Med-surg/Oncology and Observation units, providing IV/IM/oral medications, telemetry and lab/vital monitoring, and comprehensive patient education. Whether coordinating rapid response or supporting end-of-life/comfort care, I center safe, compassionate outcomes and clear communication with patients, families, and interdisciplinary teams.
