I coordinate individualized hospice care at Elysium Hospice, managing patient plans of care, nursing assessments, medication reconciliation, caregiver education, and skilled nursing visits in patients’ homes.

I also support quality assurance, maintain accurate EMR documentation, and collaborate with physicians and interdisciplinary teams to communicate changes in condition and deliver safe care.

Previously, I coordinated hospice services at Rozeroom Hospice and served as Charge Nurse at Montebello Care Center, overseeing medications, treatments, physician orders, CNA staff, infection control, and safety standards.
