I’m an RN Case Manager with 17+ years of experience in managed care, hospice, triage, and acute care. I currently provide telephonic care coordination for dual-eligible populations, bringing strengths in HRA completion, care planning, utilization management, motivational interviewing, and interdisciplinary collaboration.
Across my roles, I’ve conducted medical necessity and level-of-care evaluations, applied evidence-based clinical criteria (MCG/NCCN concepts), and translated member benefits into appropriate utilization decisions. I’ve also led complex hospice caseloads with holistic, symptom-focused planning, delivered family education and proactive interventions, and supported safe transitions of care through continuous provider and team communication.
