I’m a Registered Nurse with experience in home health and acute care, specializing in clinical assessment, care planning, and documentation to support medical necessity and reimbursement. In my current RN Case Manager role at Elara Caring, I conduct comprehensive Start of Care assessments, manage a caseload of ~20 patients per week, and prioritize clinical needs and resource utilization.

I develop individualized care plans aligned with physician orders and payer expectations, perform medication reconciliation, and continuously evaluate treatment effectiveness. I coordinate care with providers and interdisciplinary teams, maintain detailed compliant documentation in Home Care Home Base (HCHB) and Epic EMR, and escalate changes in patient condition to help prevent avoidable hospitalizations. I’m also seeking to transition into a Utilization Review role.
