What I'm looking for

I’m looking for a population health or care management role where I can drive HEDIS/Medicare quality performance, close care gaps, and coordinate CCM/TCM/AWV outreach in a collaborative, continuous-improvement environment.

I’m a Population Health Coordinator with 12 years of healthcare experience and 4+ years focused on population health operations, care management, and Medicare quality programs. I bring hands-on expertise in care gap closure, chart abstraction, clinical documentation review, and value-based care initiatives that directly support better patient outcomes.

I lead and support Medicare population health initiatives including Chronic Care Management (CCM), Transitional Care Management (TCM), Annual Wellness Visits (AWV), preventive care outreach, and chronic disease management programs. I’m experienced in patient engagement, scheduling and follow-up, medication reconciliation, symptom monitoring, chronic disease education, and post-discharge support.

I analyze medical records to validate quality metrics and obtain external records to ensure accurate quality reporting. I generate and manage outreach initiatives, coordinate interdisciplinary care across multidisciplinary teams, and support performance improvement initiatives. I also support peer-to-peer reviews, insurance appeals, PASSR assessments, and Determination of Need (DON) documentation.

I’m proficient in Epic EHR and payer portals, including insurance authorization management through multiple systems, and I’m comfortable working independently in a hybrid and remote environment while maintaining quality, productivity, and patient engagement goals. I’m motivated by measurable improvements—turning documentation and data into real, actionable care.
