What I'm looking for

I'm looking to continue improving diabetes and chronic disease outcomes through patient-centered, evidence-based care, structured workflows, remote-ready clinical support, and education for patients, families, and care teams.

I've supported diabetes care at Ballad Health, previously through Sharecare and Healthways contracts, since 2010. I collaborate with physicians to assess inpatient glycemic-management needs using American Diabetes Association standards and evidence-based protocols.

I deliver structured outpatient diabetes education that helps patients, caregivers, and families meet clinical goals and avoid complications. My work includes complex subcutaneous insulin therapies, CGM interpretation, insulin-pump troubleshooting, motivational interviewing, and SMART goal development.

I document in Epic EMR, conduct telephonic assessments, and support both in-person and remote encounters through Teams, Word, and PowerPoint. I also train nursing staff on insulin-dosing software and work to reduce hospital readmissions through patient-centered chronic disease management.
