I’m a patient-focused Registered Nurse with 8+ years of home health experience, specializing in chronic disease management and end-of-life care. I proactively assess patients through in-home and telephonic evaluations, identify early signs of deterioration, and initiate timely interventions to help prevent avoidable admissions.

I strengthen outcomes through telephonic triage, remote monitoring, and close coordination with interdisciplinary teams. I deliver skilled nursing care such as medication management, wound care, IV therapy, and symptom control, while maintaining strict Medicare/Medicaid compliance and accurate documentation in electronic medical records (including Homecare Homebase/HCHB).
