What I'm looking for

I'm looking to continue supporting individuals with complex behavioral health and medical needs through person-centered care coordination, crisis intervention, counseling, and connections to long-term community services.

At The Monroe Plan for Medical Care, I coordinate person-centered behavioral health care for members with complex medical and mental health needs. I build therapeutic relationships, assess suicide, violence, and homicidal risk, and support engagement in long-term counseling and treatment.

Previously, at Elderwood Nursing Home, I directed social work and admissions, led Social Workers completing DOH assessments, and advocated for long-term care or short-term inpatient rehabilitation. I managed counseling caseloads, developed treatment plans from psychosocial assessments, and collaborated with medical professionals on severe suicidal ideation.

My social work experience also includes St. Ann’s Nursing Home, Lifetime Assistance, and AIDS Rochester, where I provided crisis assessment, home-based counseling, case management, behavioral planning, and community referrals. I also represented individuals in Rochester City Drug Court, collaborating with judges, lawyers, and CPS.

I bring a practical, compassionate approach to connecting people and families with mental health, substance abuse, medical, grief counseling, and community supports.
