This role is designed for a results-driven healthcare professional with a passion for operational excellence and value-based care. You will lead the implementation, monitoring, and optimization of government and value-based care programs, ensuring compliance, efficiency, and measurable outcomes. Working cross-functionally, you will influence key stakeholders, develop program strategies, and support population health initiatives that improve care delivery. Accountabilities
- Serve as the subject matter expert and tactical lead for value-based care programs, including ACO REACH and other CMMI initiatives.
- Collaborate with Market Operations, Finance, Clinical Programs, and other teams to identify solutions and deploy cross-functional initiatives.
- Develop and execute tactical plans for consistent program implementation and education across markets.
- Lead design efforts in population health data reporting and care delivery enhancements.
- Manage specialist and SNF networks and act as the primary contact for ACO preferred providers.
- Conduct performance analyses, ROI assessments, and prepare reports to monitor program effectiveness.
- Ensure programs comply with regulations, are properly operationalized, and provide actionable insights.
- Maintain program databases, provider rosters, and performance tracking tools.
- Support presentations, updates, and strategy briefs for leadership, government agencies, and external partners.
Requirements
- Bachelor’s degree in public health, healthcare administration, business, or related field; Master’s preferred.
- 7+ years of relevant experience in managed care, quality improvement, or government program management.
- 3+ years in value-based care, population health, government programs, or quality improvement.
- Deep knowledge of Medicare, ACOs, and CMS Innovation models (CMMI).
- Strong project management skills with the ability to manage multiple initiatives simultaneously.
- Experience implementing, monitoring, and adjusting organization-wide programs.
- Analytical and problem-solving skills; proficiency in performance reporting and data interpretation.
- PMP or Lean Six Sigma certification preferred.
- Excellent written and verbal communication skills for diverse internal and external audiences.
- Advanced proficiency with Microsoft Office Suite and project management tools.
