About the Role:
- Conduct thorough reviews of patient charts, including histories, previous visits, labs, imaging, referrals, and medications.
- Pre-chart for scheduled patient appointments by entering structured notes, HPI templates, and clinical context into the EHR ahead of provider review.
- Identify gaps in care, relevant past medical information, and support providers in care planning through complete chart preparation.
- Collaborate virtually with providers to ensure documentation accuracy and alignment with clinical workflow.
- Maintain confidentiality and comply with HIPAA regulations at all times.
- Support documentation consistency across encounters by following provider-specific preferences and organizational templates.
- Assist with updating problem lists, medication reconciliation, and preventive care reminders when appropriate.
- Communicate with clinical teams as needed to clarify or escalate missing documentation or issues in the medical record.
About You:
- Education:
- High school diploma or GED required.
- Associate’s or Bachelor’s degree in a health-related field preferred.
- Experience:
- At least 1 year of experience as a medical scribe, clinical documentation assistant, or EHR support specialist.
- Prior experience in cardiology is strongly preferred but not required.
- Prior experience with pre-charting or working in a virtual clinical support role strongly preferred.
- Skills and Competencies:
- Proficient in medical terminology, medications, anatomy, and common disease processes.
- Strong attention to detail and organizational skills.
- Excellent written communication and data entry accuracy.
- Ability to work independently with minimal supervision.
- Comfortable using video conferencing, messaging platforms, and remote work tools.
- Reliable internet connection and secure home office environment.
Work Environment
- Fully remote position; must have access to a quiet, private HPIAA compliant workspace and stable internet.
- Must be available during clinical hours for assigned providers.
- Compliance with data privacy and security standards is mandatory.
