Expressable is a rapidly growing virtual speech therapy practice with over 200 team members on a mission to transform care delivery and expand access to high-quality services, serving thousands of clients since our inception in late 2019. About the Role:
We are seeking caring and inclusive Care Coordinators to provide top-notch customer service and support to our clients and families, ensuring a smooth and positive care experience at
Expressable. The ideal candidate is able to build rapport quickly, enjoys applying investigative skills to unusual situations, and communicates effectively with all stakeholders.

What you would be doing at Expressable
- Provides customer service to our clients and their families, assisting them with information, solving problems in collaboration with our back-office teams, and providing a great experience for the families we serve.
- Performs outreach to clients to close gaps in care and improve care plan adherence and client outcomes.
- Communicates with providers, payers, and community based organizations to resolve barriers to care: obtaining referrals, sharing clinical information with their care teams, and helping clients to access resources in their communities which may help improve their health outcomes.
- High School Diploma
- 1 years of customer service and communication (phone, email/text channels) experience
- 1 years of experience collaborating with other providers/a care team, referral coordination, accessing community-based services,
- 1 years of experience scheduling appointments
- Care Coordination in a clinical setting, medical administrative office, community health work is preferred
- Any single or combination of CHW(Community Health Worker)/Promotor(a), CHES (Certified Health Education Specialist), MSW/LCSW (Social Work) certifications (or similar formal training) is preferred.
- Proficiency in CRM systems (i.e., Salesforce, Sugar, Hubspot), Service Center systems (i.e., Salesforce, Jira, Zendesk, Freshdesk), and unified communication systems (i.e., Dialpad, Ring Central, NiceCX, Five9)
- Familiarity with: Spreadsheets (i.e., Google Sheets, Microsoft Excel), Documents (i.e., Google Docs, Microsoft Word)
- Ability to collaborate with a fully remote geographically distributed team
