What I'm looking for

I’m looking for a role in patient navigation or healthcare program coordination where I can improve care transitions and patient experience for cancer patients, collaborate with clinical teams, and uphold privacy/confidentiality through strong, compassionate communication.

I’m a Patient Navigator who helps patients and families move smoothly through care transitions, while protecting dignity, autonomy, and confidentiality. In the KEVAT Patient Navigation Program, I’ve worked to improve the patient experience through targeted communication and proactive gap-filling.

I completed 75 hours of structured training across healthcare system navigation, continuum of care, and patient support protocols across diverse clinical settings. Through SDOH-focused barrier identification and recommendations, I contributed to approximately a 30% reduction in care transition times and about a 20% improvement in patient satisfaction scores.

As part of the Patient Navigation Program - KEVAT work associated with Tata Memorial Hospital, I’ve strengthened my understanding of the healthcare structure and transitions of care, while applying frameworks around patient rights, ethics in care, advanced directives, and psychosocial support. I work with HIPAA-aligned patient privacy and confidentiality standards at the center of my approach.

Earlier, I served as a Program Coordinator at the Indian Cancer Society - Delhi, visiting government and public hospitals to provide emotional and moral support to patients and caregivers, handling internship programs and a helpline for queries, and updating and organizing training manuals. I also coordinated international volunteers between India and Cape Town, and in prior roles supported underprivileged children through teaching, administration, and fundraising.
