I’m looking to grow in medical & psychiatric social work—owning case management, psychosocial assessments, and patient navigation, while supporting community outreach or program coordination that strengthens rehabilitation and palliative care outcomes.

I’m a dedicated Medical and Psychiatric Social Work professional with experience across mental health rehabilitation, oncology patient support, community outreach, and palliative care. I bring strengths in case management, psychosocial assessment, patient counseling, and rehabilitation planning within multidisciplinary healthcare settings.

As a co-founder & trustee of a grassroots mental health advocacy and community rehabilitation foundation, I help shape vision and governance while managing fundraising compliance and strategic planning with the Board. I design outreach and psychoeducational campaigns and build partnerships with local administration, medical clinics, and community groups to expand program reach.

I’ve managed day-to-day clinical operations of a rehabilitation halfway home, coordinating multidisciplinary mental health teams and supporting resident recovery through structured assessments and individualized transition tracks. I also coordinated end-to-end oncology patient care as a patient care companion—mentoring caregivers on digital health appointment tools, organizing MDT consults, and maintaining private, HIPAA-compliant digital patient databases.
