What I'm looking for
I’m a Registered Nurse focused on providing direct, compassionate care while coordinating every step of the patient journey. Across my roles, I “implement and monitor patient care plans,” “assess and coordinates… discharge planning needs,” and ensure orders are carried out accurately.
In case management at Ascension Seton Medical Center, I coordinated and facilitated care by conducting intake interviews to determine needs and eligibility, developing individualized care plans, and connecting clients with healthcare, social services, and community organizations. I also provided emotional support and guidance to patients and their families.
At Eating Recovery Center, I develop and implement individualized care plans, educate patients about their medications and potential side effects, and collaborate with psychiatrists, psychologists, and therapists to deliver comprehensive mental-health-focused care. I also provide crisis intervention services, triage patients to be seen by a doctor, and manage patients with safety concerns.
