Plans, organizes, develops, and leads the overall operation of the assisted living neighborhood(s) in accordance with federal, state and local laws and regulations and Sunrise Senior Living standards, policies, and procedures. Champions the all-embracing quality delivery of the Resident Centered model.

Assess resident health status, develop and implement individualized care plans, and administer medications as prescribed. Provide direct patient care as needed, including wound care, vital signs monitoring, and personal hygiene assistance.

Conduct virtual consultations with patients and provide patient education on lifestyle and behavior as it relates to clinical status. Review and respond to remotely transmitted patient data and document patient communication in patient records.

Assist providers in discharge planning for members following acute episodes of care and maintain a caseload of 250+ members on a daily basis. Complete health care screenings and assess members needs, follow up post emergency and inpatient hospitalization, and submit referrals on behalf of members to appropriate state funded programs.

Delegate and supervise direct care tasks with nursing assistants and complete medication and treatment passes in accordance with State regulation and facility policies. Perform environmental rounds on assigned shift and complete charting, reports, medical forms and other documentation as required.

Provide a full continuum of behavioral health services to geriatrics, adults, and children. Monitor and report changes in patient physical or mental status, administer patient medications as prescribed by the physician, and document shift observation of patients as assigned.

Completed an Associate's degree in Licensed Practical Nursing.

Completed upper secondary education.
