What I'm looking for

I'm looking to transition my direct-care nursing experience into a remote Telehealth Nurse role focused on patient communication, clinical support, care coordination, treatment adherence, and accurate electronic documentation.

I've provided clinical support, medication management, patient education, and care coordination across skilled nursing and senior living settings, including Chateau Living Center and Sunrise Senior Living. I monitor acute and chronic care needs, identify changes in condition, communicate concerns to providers, and support treatment adherence.
