What I'm looking for

I'm looking to bring my reliability in clinical triage, medication-related care, patient and family education, and transitions of care to a role with broader care coordination or case management responsibility.

I'm currently delivering hospice care at Hospice of Southwest Ohio, providing medication administration, wound care, comfort-focused treatment, crisis support, and education for patients and families.

At TriHealth, I managed outpatient triage, scheduling, vaccinations, blood draws, medication administration, prior authorizations, and EPIC documentation. I helped keep communication aligned between patients, providers, and clinical staff.

As an LPN Charge Nurse with IntelyCare and a home health nurse supporting Ohio Medicaid patients, I've cared for medically complex pediatric, geriatric, long-term-care, and post-acute populations. My work includes clinical assessment, escalation, ventilator management, G-tube care, staffing coordination, and accurate case documentation.

Across more than 20 years in healthcare, I've partnered with physicians, case managers, social workers, families, and interdisciplinary teams to support safe, consistent care. I'm ready to bring my experience in clinical triage, transitions of care, and patient communication to broader care coordination or case management work.
