What I'm looking for

I’m looking for a leadership-forward clinical social work role where I can supervise, coordinate complex cases, and collaborate with physician-led multidisciplinary teams to improve access to care and discharge outcomes for medically and socially vulnerable patients.

I’m a Licensed Clinical Social Worker and Qualified Supervisor who organizes priorities, independently manages workload, and meets multiple deadlines while partnering with physician-led multidisciplinary teams.

In my current role with the TAMPA VA HEALTHCARE SYSTEM (SABAL PARK VA PATIENT ALIGNED CARE TEAM / PACT), I lead daily care coordination and consulting, and I negotiate and coordinate support plans to reduce barriers like access to care and discharge needs.

Across inpatient and community settings, I’ve provided direct clinical social work services for adjustment to illness, advance directive support, elder abuse (verbal, physical, financial, neglect, self-neglect, exploitation), intimate partner violence, grief, end of life planning, and suicide risk assessment, along with specialized coordination for long-term care and veteran-directed services.

I also bring hands-on expertise in psychosocial assessments, discharge planning, caregiver support assessments, guardianship/payee/fiduciary needs, and arranging home services such as home health, visiting nurse, home infusion, hospice, transportation, and DME—plus insurance authorization and coordination with community resources.
