What I'm looking for

I’m looking for a role where I can deliver compassionate psychosocial assessments, coordinate safe transitions of care, and work with interdisciplinary teams—using solution-focused and crisis interventions to support patients and families.

I’m an MSW-trained, LCSW-licensed social worker who brings a strong foundation in crisis care, psychosocial assessment, and safe care transitions. I focus on helping patients and families navigate complex needs with clarity, empathy, and practical next steps.

In my current LMSW role, I conduct comprehensive psychosocial evaluations for patients transitioning from acute care to home or community-based services. I assess post-acute needs (including rehab, home health, DME, and long-term care), educate patients and families on Medicare/Medicaid and state waiver programs, and provide bereavement counseling and emotional support. I also facilitate weekly interdisciplinary rounds and deliver caregiver education to reduce burnout and improve safety and engagement.

Previously at HCA Hospice and Family Care, I assessed patients, families, and caregivers’ psycho-social needs related to terminal illness and environment safety. I communicated findings to the interdisciplinary team, supported discharge planning for the general in-patient population, and acted as a liaison between patients/families and community agencies while supporting individualized plans of care.

Before that, I worked as a Transitional Coordinator at The Harris Center and a Treatment Plan Specialist/COC Specialist at Tri County Behavioral Health, coordinating access to psychiatric/medical care for juveniles and coordinating complex behavioral and physical healthcare needs for adults and adolescents. I also completed mobile crisis work, providing crisis intervention, de-escalation and stabilization, safety planning, and brief CBT/solution-focused/trauma-informed interventions—often in collaboration with law enforcement, EMS, hospitals, schools, CPS, and community partners.
