As Director of Case Management, I oversaw case managers and payor relation specialists, ensured regulatory-compliant care, and acted as a liaison between medical providers and care teams to streamline patient navigation. I have a track record of reducing hospital length of stay by educating staff on resources and coordinating multidisciplinary care for complex patients.
