Lisa Marie Dimitris

I'm directing organization-wide patient outcome initiatives at Clearsky Health, using healthcare data and KPIs to improve quality metrics and patient satisfaction. I present outcome reports and recommendations to leadership, educate staff and physicians on functional outcome measurement, and manage case management.

At Advocate Aurora, I've served as a Nurse Care Manager, Nurse Educator for the Davos Alzheimer's Collaborative Project, HELP Program Coordinator, and Neurology Department Lead. I launched delirium-prevention programming, developed dementia education for primary care providers, coordinated Project ECHO, and supported neurology patients through treatment, triage, and follow-up care.

I've also provided wellness consultation in senior living and delivered independent nursing care in a detention-center environment. My work includes medication administration, clinical assessment, documentation, staff education, and patient and family education.

I bring an MSN, RN background along with experience collaborating across providers, staff, volunteers, and leadership to strengthen patient care and measurable outcomes.
