At Elevance Health, I conduct home and community assessments for Medicaid and Medicare members, develop and revise managed long-term services and supports care plans, and coordinate interventions that help prevent emergency visits and hospital readmissions.

I've delivered nursing care across cardiac telemetry, rehabilitation, long-term care, oncology, intensive care, and medical-surgical settings. I bring experience in clinical assessments, service authorization, medication reconciliation, wound and IV care, patient advocacy, and safe transitions of care for patients and families.
