At Pinero Preventative Care, I evaluate patients across the lifespan and develop care plans for acute and chronic conditions. I manage complex cases including CHF and COPD exacerbations, diabetic and hypertensive emergencies, and adjust medication regimens in collaboration with pharmacists.
At InnovaCare (Orlando Family Physicians), I managed a Medicare Advantage panel and reviewed hospital records to identify documentation gaps and obtain provider clarification. I also coordinated post-discharge follow-up and medication reconciliation, and supported HEDIS measures and care-gap closure.
