I’m a Registered Nurse with 17+ years of experience providing complex care management, patient education, and care coordination across acute care, community health, and population-based settings. BSN-prepared, I build, implement, and evaluate individualized care plans that address physical health needs, barriers to care, and social determinants of health (SDOH).
Across my most recent role, I support members through telephonic and digital outreach, thorough documentation, and regulatory compliance. I identify barriers to care (including access and psychosocial challenges), collaborate with providers and interdisciplinary teams, and help patients and caregivers stay aligned with disease process education and care plan goals.
Before that, I developed and delivered individualized plans in Labor & Delivery (Level III NICU) and served as a School Nurse, connecting families to community resources and ensuring continuity of care.
