I’m seeking remote or hybrid roles in care coordination, utilization management, maternity programs, or nurse education—where I can apply telehealth case management, patient teaching, and proactive planning to improve outcomes with interdisciplinary teams.

I’m an MSN-prepared registered nurse focused on telehealth care coordination, high-risk obstetrical case management, and patient education. I combine complex clinical decision-making with a teaching mindset to help patients and families improve self-management and outcomes.

In telehealth and Patient Aligned Care Teams work at Northern Indiana Veterans Affairs, I enrolled and supported patients with CHF, COPD, diabetes, and hypertension using remote monitoring programs. I provided education, coaching, and proactive care planning, collaborating with providers and PACT teams and leading team huddles to identify patients needing additional support.

As a High-Risk Obstetrical Case Manager at Anthem, I helped implement the federally funded Pregnancy Promise Program for opioid-affected pregnancies. I conducted assessments, care planning, and ongoing case management while connecting patients to community resources and coordinating OB care to support maternal outcomes.
