At Commonwealth Care Alliance, I support members with chronic disease management and end-of-life care through scheduled telephonic outreach. I also conduct comprehensive assessments and coordinate follow-up based on each member’s needs.
I perform in-person and telephonic MDS assessments in accordance with MassHealth requirements. I coordinate healthcare and community-based services, including transportation, LTSS, and hospice care.
At Massachusetts General Hospital, I provided nursing care to medically complex patients on a medical-surgical and telemetry unit. I worked with case management on safe discharge planning and post-discharge resources.
Earlier, I provided medical-surgical, telemetry, orthopedic, and surgical nursing care in hospital settings, and nursing care in acute rehabilitation and long-term care. My experience also includes complex wound care, IV care, and precepting RN students and newly hired nurse residents.
