I’m an experienced Registered Nurse with 8+ years of progressive healthcare experience across home health, acute/progressive care, leadership, rehabilitation, and patient advocacy. I specialize in case management, OASIS documentation, and coordinating interdisciplinary care to improve outcomes.

In my current role as a Registered Nurse Case Manager, I manage a caseload of approximately 35–40 patients while meeting productivity expectations of 32–36 patient care points weekly. I perform OASIS assessments (SOC, ROC, recertifications, and discharge evaluations) and collaborate closely with physicians to reduce avoidable hospital readmissions.

I bring deep wound care expertise, including Wound VAC therapy and medication reconciliation. I maintain documentation in Homecare Homebase while ensuring Medicare compliance and regulatory compliance throughout the care continuum.

Previously, I served as a home health case manager, an ADON / Infection Prevention Nurse, and an RN Unit Manager—leading infection prevention initiatives, compliance audits, and nursing operations. My clinical foundation includes Progressive Care and ICU Step-Down nursing for medically complex and critically ill patients.
