Support patients transitioning from hospital to home by coordinating specialty and home infusion services, partnering with healthcare providers and internal teams to ensure seamless access to therapy and positive patient outcomes.

Requirements

  • Coordinate the continuum of care for patients receiving Specialty IV Therapy, Home IV Therapy, Specialty Pharmacy, or Enteral Nutrition services.
  • Collaborate with hospitals, clinics, physicians, and home health agencies to ensure timely and effective home-based care.
  • Manage the full referral lifecycle, including insurance verification, home health scheduling, and coordination with operational teams.
  • Maintain accurate documentation and ensure patient confidentiality at all times.
  • Participate in required meetings, in-services, and training activities.
  • Comply with established administrative, HR, and organizational policies.
