THE OPPORTUNITY:

Medical Home Network (MHN) is seeking a highly skilled Director of Population Health to lead initiatives aimed at improving patient care, enhancing clinical outcomes, and optimizing value-based care (VBC) performance. This role will combine expertise in population health with operational leadership, working closely with cross-functional teams to drive meaningful change in healthcare delivery. As a key member of the leadership team, the Director of Population Health will work with Data Analytics (MDAT), Financial Planning Analysis (FPA), and Clinical Operations to identify opportunities for improving care and driving business performance.

WHAT YOU CAN LOOK FORWARD TO:

  • Lead population health and business intelligence (BI) functions to improve performance across four key areas:
  • 1. Patient outcomes and resultant total cost of care improvement
  • 2. Appropriate documentation aligned with patient management
  • 3. Quality improvements focused on delivering excellent care
  • 4.Patient retention and engagement to foster strong primary-care based longitudinal care/relationships
  • Develop a strategic view of population health opportunities and drive insights into actionable operations.
  • Work closely with the MDAT team to analyze claims and financial data, identifying high-impact opportunities and developing actionable insights.
  • Partner with MHN Clinical Operations team to develop and implement population health interventions and programming to impact population health outcomes and performance.
  • Work cross functionally to optimize contract performance and outcomes.
  • Influence the creation of dashboards and data visualizations that drive meaningful insights and decision-making (expertise in Power BI and Tableau for usage, not design).
  • Collaborate with executive leadership to define and execute population health strategies aligned with MHN’s mission and business objectives.
  • Provide leadership and management to a growing team, fostering a culture of accountability and excellence.

WHAT YOU’LL NEED TO SUCCEED:

  • Bachelor’s degree required. MBA strongly preferred. Consulting background a plus.
  • 3+ years of experience in population health operations and data analytics, with 5-7+ years in healthcare.
  • Proven experience working in a value-based care (VBC) delivery or enablement environment.
  • Strong business and analytical mindset with the ability to translate data into strategy and operational improvements.
  • Expertise in Excel and data visualization tools (Power BI or Tableau).
  • Ability to roll up your sleeves and drive outcomes in a fast-paced environment.
  • Ability to identify strategic opportunities, develop actionable plans, and execute effectively.
  • Experience collaborating with cross-functional teams, including data analytics, finance, and clinical operations.
  • Strong business acumen and problem-solving skills.
  • Consulting experience a plus, specifically direct operations within a healthcare system.
  • Management experience strongly preferred.
