We achieve this by transforming community-based organizations such as shelters, food pantries, and rehabilitation facilities into accessible healthcare sites, leveraging telemedicine and in-person engagements to meet patients where they are. Our innovative AI-native technology platform, Arc, is central to our approach. Arc empowers our multidisciplinary care teams—comprising community health workers, registered nurses, behavioral health clinicians, and practitioners—to deliver personalized and coordinated care efficiently. This platform automates time-consuming administrative tasks like documentation, scheduling, and resource coordination, allowing our teams to dedicate more time to direct patient interaction and care. By focusing on prevention and proactive engagement, Pair Team's model has demonstrated significant success in reducing emergency room visits and hospital admissions, leading to substantial cost savings and, more importantly, dramatically improved quality of life for the individuals we serve.

Our commitment to innovation extends to our operational structure and community partnerships. Recently transitioning to a Public Benefit Corporation (PBC), Pair Team has further solidified its dedication to creating positive social impact while building a more effective and equitable safety-net healthcare system. We partner extensively with local health centers, social service organizations, and Medicaid plans, training staff within these organizations to become community health workers. This collaborative approach not only expands our reach but also builds upon the existing trust and relationships these organizations have within their communities. The technology we provide supports initiatives like CalAIM (California Advancing and Innovating Medi-Cal), which aims to close health equity gaps by advancing whole-person care. Pair Team's platform is designed to be care plan and workflow-oriented, incorporating an internally-developed AI copilot using Large Language Models to streamline administrative and coordination workflows. This enables our care teams and partners to provide cohesive, comprehensive support that addresses clinical needs alongside critical social factors such as housing instability and food access. Through our Community Health Platform and our AI-enabled medical group, we are scaling this high-quality, whole-person care model nationally, striving to make transformative healthcare technology and services equally accessible to all, particularly those who have historically faced the greatest barriers to care.
