At Ayden Toledo, I coordinate Medicare and utilization review meetings, complete timely MDS assessments, monitor quality measures, and support care planning and reimbursement.

I manage case-management responsibilities for up to 90 patients, collaborating with nursing, physicians, therapy teams, case managers, and insurance companies to facilitate safe discharges home. I also review documentation, educate staff, coordinate care plans, and help ensure clear communication around discharge readiness and available resources.

Previously at Glass City Health and Rehab, I combined nurse supervisor, MDS, case management, medical records, and interim unit manager responsibilities for skilled and long-term care patients. I trained nurses and STNAs, administered medications and treatments, supported wound care, maintained medical records, monitored certifications, and coordinated treatment plans with clinical teams.

My nursing background also includes behavioral health, detox, medication administration, tube feeding, ICD-10 coding, HIPAA-compliant records management, and patient education.
