Shawn Tan

At United Cerebral Palsy of the North Bay, I support program leadership in delivering person-centered services for adults with cerebral palsy and related developmental disabilities. I conduct intakes and evaluations, develop Individual Service Plans, coordinate activities, and support staff training and scheduling.

I've led employment and vocational services at Adaptive Learning Center, All About Ability, TkMomentum, and Telecare Corporation, helping people with disabilities, mental health needs, and homelessness navigate employment, rehabilitation, and community resources. My work includes client assessments, job coaching, case management, employer collaboration, program coordination, and staff supervision.

I'm also an educator and mentor, having provided K–12 tutoring through California College Corps and peer mentoring at Woodland Community College. I bring experience in individualized learning, motivational interviewing, crisis intervention, care coordination, and supporting underserved communities.
