I'm currently a Care Manager at Bluebird Care Services in London, where I develop nutrition-focused, person-centred care plans for elderly individuals and children with SEND.
I conduct MUST and ADL assessments for people with dementia, diabetes, hypertension, and dysphagia, and work with GPs, occupational therapists, and social workers to provide integrated care. I also run monthly audits to close documentation gaps against CQC standards.
As an independent nutrition consultant, I design personalised plans for weight management, disease management, and sports performance.
