What I'm looking for

I’m looking for a role in patient support and health coaching where I can deliver compassionate, evidence-informed care—especially through digital/virtual support—collaborate with multidisciplinary teams, and help clients achieve sustainable behaviour change and better outcomes.

I am a Registered Social Worker (RSW) with a Master of Social Work (MSW) and experience supporting individuals, families, and communities across healthcare, child protection, and social welfare settings. I provide psychosocial assessment, patient support, behaviour change coaching, counselling, care coordination, and case documentation, helping clients access appropriate health and social services.

My experience includes supporting patients and vulnerable populations through psychoeducation, follow-up care, multidisciplinary collaboration, and medication adherence counselling to improve health and well-being. I am passionate about digital health, preventive care, chronic disease management, and empowering people to make sustainable lifestyle changes through compassionate, person-centred support.
