What I'm looking for

I’m looking for a role where I can lead care coordination for complex psychiatric and substance-use patients—streamlining admissions, managing authorizations, and preventing avoidable hospital visits through compassionate, evidence-based practice.

I’m an experienced, compassionate Registered Nurse with 9+ years in psychiatric, addiction medicine, and intake coordination. I focus on care coordination, case and utilization management, and crisis intervention—especially for co-occurring and substance use disorders.

Most recently as a Nurse Liaison (Remote), I coordinated hospital inpatient detox and rehab admissions and ED warm handoffs across Gateway facilities. I conducted extended screening and biopsychosocial assessments daily, determined level of care using ASAM and InterQual criteria, managed prior authorizations with multiple MCOs, and served as a single point of contact to expedite admissions. I also operated a MAT clinic one evening weekly with Sublocade, Brixadi, and Vivitrol, and I designed a tracking tool to monitor hospital referrals and admissions.

In my transition care management role, I help high-risk multi-chronic patients avoid frequent hospital visits by following up after ED and inpatient discharges and educating patients on appropriate AHN resources. Earlier in my career as an RN Supervisor/ADON at Southwood Psychiatric Hospital, I supervised operations of a 74-bed child and adolescent psychiatric hospital, led interdisciplinary collaboration, and trained staff in crisis management and restraint interventions.
