What I'm looking for

I'm looking to support and enrich patient experiences through individualized care, advocacy, collaborative care planning, and personalized objectives that improve health outcomes.

I'm currently a Registered Nurse Case Manager/Supervisor at 1st Care Home Health, coordinating individualized care plans, assessments, referrals, discharge planning, and patient education. I advocate for patients’ rights while collaborating with interdisciplinary teams to support continuity of care and improve outcomes.

Previously, I provided home-based nursing care at LHC Group and cared for women, newborns, and families at SOVAH. I bring experience in clinical documentation, maternal and newborn assessments, high-risk IV medications, care coordination, and patient-centered advocacy.
