Julissa Deleon

I’m a Central Intake Coordinator with experience coordinating patient referrals and managing admissions needs across hospital, SNF, and hospice requests. I review inquiries to determine whether we can meet patients’ and families’ needs, and I connect with case managers for current updates.

In my current role as an Admissions Coordinator, I handle facility information requests, tour families of potential patients, and communicate clearly with the nurses’ station when patients are accepted. I also support the admission process by providing required documentation guidance and ensuring admission agreement packet completion.

Previously and concurrently, I’ve supported medical records workflows—uploading records to patient charts electronically, ensuring proper physician/signatory documentation, and handling faxed materials such as discharge summaries. I bring strong attention to detail, HIPAA awareness, and reliable administrative execution to every step of intake and records management.
