At Sunnyvale gardens and Vasona Creek Healthcare Center, I assist the DON with daily clinical operations and maintain survey readiness. During the DON’s absence, I served as interim DON.
As a Case Manager under the Stanford Transplant team and Sutter Health attending, I managed a caseload of 35+ skilled patients and worked on Stanford’s new transplant program, including triple organ transplants with safe discharge to the community.
At Covenant Care/ Spyglass Healthcare, I worked as an MDS Coordinator, completing MDS assessments and care plans and participating in IDT conferences. I also maintained the QM and supported QRP assessment; the facility had never had below a 5-star rating.
I currently work as an Infection preventionist and Lead Kaiser Case manager. My nursing credentials include LVN, BLS, CPR, RAC-CT, MOCA certification, and Wound Care Certification: PWS.
