I’m an innovative, patient-focused Registered Nurse with extensive experience in care management, palliative care, hospice, triage, and outpatient services. As Director of Care Management in the Advanced Illness Management (AIM) model, I built and defined a new leadership role and lead a fully remote interdisciplinary team of Registered Nurses and Social Workers.
