What I'm looking for

I’m looking for a leadership role where I can build remote, workflow-driven care models in palliative settings, partner with interdisciplinary teams, and improve quality outcomes for high-risk, underserved patients.

I’m an innovative, patient-focused Registered Nurse with extensive experience in care management, palliative care, hospice, triage, and outpatient services. I’m a proven leader who thrives in evolving environments—especially building and scaling remote care models. I stay calm under pressure, navigate ambiguity, and develop structured, efficient workflows that improve patient and team outcomes.

As Director of Care Management in the Advanced Illness Management (AIM) model, I built and defined a new leadership role and lead a fully remote interdisciplinary team of Registered Nurses and Social Workers. I monitor productivity and performance across multiple non-integrated systems, provide coaching, education, and professional development, and strengthen communication and alignment between local branches and remote teams. I also designed and implemented productivity tracking methods across non-interfacing platforms while supporting care delivery for a high-risk palliative population with complex needs and limited resources.

Previously, as an AIM Care Manager, RN, I delivered remote care management support across multiple regions—completing clinical assessments and documentation aligned with care pathways and escalating abnormal findings to Nurse Practitioners. I also worked as a Triage Nurse providing symptom management guidance, clinical decision support, and emotional support while coordinating urgent staffing needs. Earlier in my career, I served as a Quality Care Analyst reviewing medical records for compliance, quality, and reimbursement accuracy, and as a Case Manager managing complex caseloads and crisis care coordination.
