Role Overview
We are seeking a compassionate and skilled Hospice Registered Nurse to join our dedicated team. In this fully remote role, you will provide high-quality, patient-centered care to individuals in hospice settings. Your clinical expertise will be essential in managing patient symptoms, coordinating care, and providing support to families during challenging times.
Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs in order to create individualized care plans.
- Administer medications and treatments as prescribed, monitoring patients for side effects and effectiveness.
- Educate patients and families about the hospice process, including symptom management, advance care planning, and available resources.
- Collaborate with interdisciplinary teams to ensure holistic care delivery and to address evolving patient needs.
- Document all patient interactions and updates accurately within the electronic medical record (EMR) systems.
- Provide emotional support and counseling to patients and families, fostering a compassionate care environment.
Required and Preferred Qualifications
Required:
- Current RN license in good standing with the state board of nursing.
- Minimum of 2 years of nursing experience, with a focus on palliative care or hospice preferred.
- Strong clinical assessment skills and the ability to manage complex patient cases.
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams.
Preferred:
- Experience in home health, hospice, or palliative care settings.
- Certification in hospice and palliative nursing (CHPN) or willingness to obtain within a specified timeframe.
Technical Skills and Relevant Technologies
- Proficiency with electronic health record (EHR) systems, such as Epic, Cerner, or similar.
- Familiarity with telehealth technologies for remote patient assessments and follow-ups.
- Experience with symptom management tools and patient monitoring equipment.
Soft Skills and Cultural Fit

- Ability to work independently while also being a proactive team player.
