Role Overview
We are seeking a strategic and compassionate Director of Hospice Social Work to lead our dedicated team in providing exceptional end-of-life care. In this pivotal role, you will oversee the holistic care strategies and ensure compliance with regulatory standards while championing the emotional and psychological needs of our patients and their families.
Responsibilities
- Lead and manage the hospice social work team, providing guidance on clinical best practices, ethical considerations, and emotional support strategies.
- Develop and implement care plans that address the psychosocial needs of patients and families, ensuring culturally competent support tailored to diverse backgrounds.
- Collaborate with interdisciplinary teams to facilitate comprehensive care coordination and promote effective communication between healthcare providers and families.
- Monitor compliance with state and federal regulations relevant to hospice services, ensuring policies are updated and implemented effectively.
- Conduct regular training and development sessions for social work staff, fostering a culture of continuous learning and professional development.

Required and Preferred Qualifications
Required:
- Master's degree in Social Work (MSW) from an accredited institution.
- 5+ years of experience in hospice or palliative care settings, with at least 3 years in a leadership role.
- Current state licensure as a social worker (LCSW or equivalent).
- Demonstrated knowledge of hospice regulations, Medicare guidelines, and best practices in end-of-life care.
Preferred:
- Experience with program development and implementation in a healthcare setting.
- Familiarity with electronic health record (EHR) systems and documentation standards in hospice care.
- Certification in palliative care or hospice social work (e.g., ACHP-SW).
Technical Skills and Relevant Technologies
- Proficient in Microsoft Office Suite and EHR systems.
