Role Overview
We are seeking a compassionate and skilled Lead Home Hospice Aide to join our dedicated team. In this pivotal role, you will be responsible for providing high-quality personal care to patients in their homes, ensuring they receive the support and comfort they need during this critical time. You will lead a team of aides, coordinate care plans, and collaborate with healthcare professionals to enhance the quality of life for our patients and their families.
Responsibilities
- Provide hands-on personal care to hospice patients, including assistance with daily living activities such as bathing, grooming, and meal preparation
- Lead and mentor a team of home hospice aides, fostering a supportive and collaborative work environment
- Develop and implement individualized care plans in coordination with the healthcare team, ensuring patient needs are met
- Communicate effectively with patients, families, and healthcare professionals to provide updates on patient conditions and care plans
- Conduct regular assessments of patient needs and adjust care plans as necessary to ensure optimal quality of care
- Maintain accurate documentation of patient care activities and progress in compliance with regulatory standards
Required and Preferred Qualifications
Required:
- Certification as a Home Health Aide or similar role
- 2+ years of experience in a hospice or home health setting
- Strong understanding of patient care best practices and hospice philosophy
- Excellent communication and interpersonal skills
- Ability to work independently and manage a team effectively
Preferred:
- Experience in a lead or supervisory role within a healthcare setting
- Knowledge of local healthcare resources and community services
- CPR and First Aid certification
Technical Skills and Relevant Technologies
- Proficiency in electronic health record (EHR) systems and patient care documentation
- Basic understanding of medical terminology and healthcare practices
- Familiarity with telehealth technologies and remote patient monitoring tools
