Role Overview
We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our dedicated team, where you will play a vital role in providing high-quality, patient-centered care to individuals with serious illnesses. Responsibilities
- Conduct thorough assessments and develop individualized care plans tailored to the unique needs of patients with complex medical conditions.
- Provide expert symptom management, including pain control, psychological support, and assistance with decision-making processes related to treatment options.
- Foster strong relationships with patients and their families, ensuring open communication and support throughout the care journey.
- Collaborate with healthcare teams, including physicians, social workers, and chaplains, to coordinate care and address holistic patient needs.
- Document patient care accurately and timely in electronic health records, ensuring compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance the standard of palliative care across the organization.
Required and Preferred Qualifications
Required:
- Master's degree in Nursing with a focus on Palliative Care, Advanced Practice, or a related field.
- Current Nurse Practitioner licensure in the state of practice.
- Certification as a Palliative Care Nurse Practitioner (e.g., ACHPN).
- Minimum of 3 years of clinical experience in acute care or palliative care settings.
- Strong understanding of pain management principles and advanced care planning.
Preferred:
- Experience in a fully remote work environment.
- Familiarity with telehealth platforms and digital health tools.
- Demonstrated ability to work independently while remaining a collaborative team member.
