Role Overview
We are seeking a compassionate and skilled Lead Hospice Aide to join our dedicated team, providing high-quality personal care and support to patients in their homes. This role is pivotal in ensuring patient comfort and dignity during their hospice journey, while also mentoring and guiding other aides in best practices and care standards.
Responsibilities
- Deliver personalized caregiving services, including assistance with daily living activities, personal hygiene, and mobility support
- Oversee the care provided by hospice aides, ensuring adherence to established care plans and protocols
- Collaborate closely with registered nurses, social workers, and other healthcare professionals to develop and implement comprehensive care plans
- Provide emotional support and companionship to patients and their families, fostering a comforting and supportive environment
- Conduct regular assessments of patient needs and report changes in condition to the healthcare team
- Assist in training and mentoring new hospice aides, promoting a culture of excellence and continuous improvement
Required and Preferred Qualifications
Required:
- Certification as a Home Health Aide or equivalent
- Minimum of 2 years of experience in a hospice, home health, or long-term care setting
- Strong understanding of hospice philosophy and commitment to end-of-life care
- Ability to communicate effectively and compassionately with patients and families
- Basic knowledge of medical terminology and patient care practices
Preferred:
- Experience in a leadership or supervisory role
- Certification in CPR and First Aid
- Experience with electronic health records (EHR) systems
Technical Skills and Relevant Technologies
- Proficiency in using electronic documentation systems for patient care
- Ability to utilize medical equipment such as wheelchairs, lifts, and monitoring devices
- Familiarity with basic health assessment tools and techniques
