Role Overview
We are seeking a detail-oriented and compassionate Assistant Hospice Plan Administrator to support our hospice care team in delivering high-quality patient care. In this fully remote role, you will play a critical part in coordinating patient care plans, ensuring compliance with regulations, and assisting the hospice team in achieving optimal outcomes for patients and their families.
Responsibilities
- Assist in the development and implementation of individualized hospice care plans based on patient needs and preferences
- Coordinate communication between healthcare providers, patients, and families to ensure seamless care delivery
- Maintain accurate and up-to-date patient records in compliance with federal and state regulations
- Support the hospice team in monitoring patient progress and adjusting care plans as necessary
- Prepare and participate in interdisciplinary team meetings to discuss patient care strategies and updates
- Contribute to quality assurance and performance improvement initiatives within the hospice program
Required and Preferred Qualifications
Required:
- Associate's degree in healthcare administration, nursing, or related field
- 1+ years of experience in a healthcare setting, preferably in hospice or palliative care
- Strong organizational skills and attention to detail
- Excellent verbal and written communication skills
- Familiarity with electronic health records (EHR) systems
Preferred:
- Bachelor's degree in healthcare administration or a related field
- Experience with care planning and case management in hospice or home health
- Knowledge of regulatory requirements and standards for hospice care
Technical Skills and Relevant Technologies
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and EHR systems
- Ability to analyze data and generate reports related to patient care and outcomes
- Familiarity with telehealth technologies and virtual communication platforms
