Role Overview
We are seeking a compassionate and experienced Lead Home Health Aide to join our dedicated team in providing exceptional in-home care services. In this pivotal role, you will coordinate and oversee the delivery of personalized health care services, ensuring that our clients receive the highest level of support in the comfort of their own homes.
Responsibilities
- Lead, mentor, and train a team of home health aides in delivering quality care, while fostering a supportive and collaborative environment.
- Develop and implement individualized care plans based on clients' needs and preferences, ensuring compliance with regulatory standards.
- Conduct regular assessments of clients' health and well-being, adjusting care plans as necessary to optimize outcomes.
- Serve as a primary point of contact for clients and their families, addressing concerns and facilitating communication with healthcare professionals.
- Monitor and document client progress, maintaining accurate records and ensuring continuity of care.
- Provide hands-on assistance with daily living activities, including personal hygiene, meal preparation, and mobility support.
Required and Preferred Qualifications
Required:
- High school diploma or equivalent; certification in home health care or nursing assistance preferred.
- 3+ years of experience in home health care or a similar caregiving role, with at least 1 year in a leadership position.
- Comprehensive knowledge of patient care standards, safety protocols, and best practices in home health care.
- Strong interpersonal and communication skills, with the ability to build trusting relationships with clients and their families.
Preferred:
- Experience working with diverse populations and understanding of cultural sensitivities in caregiving.
- Familiarity with healthcare software and electronic medical records (EMR) systems.
- CPR and First Aid certification.
Technical Skills and Relevant Technologies
- Proficiency in using technology for client monitoring and communication, including telehealth platforms.
- Basic knowledge of medical terminology and documentation practices.
