Role Overview
We are seeking a compassionate and experienced Hospice Social Work Supervisor to lead our dedicated team of social workers in providing holistic, patient-centered care. In this pivotal role, you will oversee the social work division, ensuring the delivery of high-quality psychosocial support to patients and their families during their end-of-life journey. You will play a key role in developing care plans, coordinating services, and advocating for patient needs while fostering a collaborative environment that promotes professional growth and development.
Responsibilities
- Supervise and support a team of social workers, providing guidance on complex cases and ensuring adherence to ethical standards and best practices.
- Conduct regular team meetings and training sessions to enhance team performance and service delivery.
- Collaborate with interdisciplinary teams to develop comprehensive care plans that address the emotional, social, and spiritual needs of patients and families.
- Assess patient needs and provide appropriate interventions, counseling, and resources to enhance their quality of life.
- Ensure compliance with regulatory requirements and agency policies related to social work practice in hospice care.
- Advocate for patients and families within the healthcare system, facilitating access to necessary services and support.
Required and Preferred Qualifications
Required:
- Master's degree in Social Work (MSW) from an accredited program.
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
- 5+ years of experience in social work, with at least 2 years in a supervisory role within a healthcare or hospice setting.
- Strong knowledge of hospice care principles, psychosocial assessment, and support services.
- Exceptional communication and interpersonal skills, with the ability to build rapport with patients, families, and interdisciplinary teams.
Preferred:
- Experience with bereavement support and grief counseling.
- Familiarity with community resources and support services available for hospice patients and their families.
- Certifications in palliative care or hospice social work.
Technical Skills and Relevant Technologies
- Proficiency in electronic medical records (EMR) systems and documentation software.
- Ability to utilize assessment tools and psychosocial interventions effectively.
- Familiarity with telehealth platforms for remote patient support.
