Role Overview
We are seeking a dedicated Foster Care Case Manager to join our team, responsible for coordinating and managing the care of children in the foster care system. This role is critical in advocating for the well-being of children and ensuring they receive the necessary support and services to thrive in a safe and nurturing environment.
Responsibilities
- Conduct comprehensive assessments to develop individualized service plans for each child in foster care, ensuring alignment with their unique needs and circumstances.
- Facilitate communication and collaboration among foster families, biological families, social workers, and other stakeholders to support the child's best interests.
- Monitor the progress of children in foster care, conducting regular visits and evaluations, and adjusting care plans as needed.
- Provide guidance and support to foster families, equipping them with resources and training to meet the emotional and developmental needs of children.
- Document case notes and maintain accurate records in compliance with state regulations and agency policies.
- Advocate for children's rights and needs within the foster care system, ensuring they receive appropriate educational, medical, and therapeutic services.
Required and Preferred Qualifications
Required:
- Bachelor's degree in Social Work, Psychology, or a related field.
- 2+ years of experience in case management or working with children and families in a social services setting.
- Strong understanding of child welfare laws and foster care regulations.
Preferred:
- Master's degree in Social Work or a related field.
- Experience working in foster care or adoption services.
- Knowledge of trauma-informed care practices.
Technical Skills and Relevant Technologies
- Proficient in case management software and electronic health records.
- Strong analytical skills to assess complex situations and develop actionable plans.
- Ability to utilize various communication tools for remote case management.
